Requirements
- Be registered in the parish. If you are registered in another parish, please bring a letter from your pastor granting authorization.
- Birth certificate of the person concerned.
- Certificate of Baptism (Proof of Baptism).
- Complete the application form.
- Come to the office to review the documentation once everything is ready:
- Submit the completed application form
- Schedule the sacrament (The date will depend on the priest’s availability)
